Bowie didn't say deliver many clear messages in his songs, but this was as clear as it gets. He wants to stay clean tonight, but he bought some dope ("stuck with a valuable friend") and so he indulges, then he just wants to be sober (come down) and the frustration makes him consider suicide.

I must have listened to this version 30 times today. Something about is just so epic.
I was fortunate enough to see him on the Glass Spider Tour in Atlanta in September 1987. Not his best tour I hear but I am fortunate to have seen him.
Such a loss for music. The state of music these days true artists such as Bowie is a rarity anymore.
